New insights into fetal atrioventricular block using fetal magnetocardiography.

چکیده
outine fetal arrhythmia assessment typically involves the se of echocardiographic modalities that demonstrate mehanical events from which electrophysiological events are nferred (1). While M-mode and Doppler-based techniques ave been used successfully to diagnose third-degree atrioentricular block (AVB), second-degree AVB, and, more ecently, first-degree AVB, our evaluation has been limited o atrial and ventricular rates (2–4), gross P-R interval stimates using atrial-ventricular activity time intervals (5), nd rarely detected ventricular ectopy. Although fetal elecrocardiogram (ECG) has been shown to be more sensitive han echocardiogram-based techniques in detecting firstegree AVB in anti–Ro-positive pregnancies (6), it has imited use in the evaluation of fetal heart rates out of the ormal range, is more difficult to acquire at 28 to 32 weeks, nd involves signal averaging, all limiting its clinical pplication.

Prenatal Diagnosis of Atrioventricular Block and QT Interval Prolongation by Fetal Magnetocardiography in a Fetus with Trisomy 18 and SCN5A R1193Q Variant
We report a case of fetal trisomy 18 with SCN5A R1193Q variant that presented with sinus bradycardia, 2 : 1 atrioventricular block (AVB), and QT interval prolongation. These complex arrhythmias were diagnosed by fetal magnetocardiography combined with ultrasound findings. Advanced AVB and ventricular arrhythmias were confirmed after birth. Genetic testing of the baby revealed a SCN5A R1193Q var...
متن کاملMagnetocardiography-guided management of an unusual case of isoimmune complete atrioventricular block complicated by ventricular tachycardia.
A fetus who was diagnosed at 25 weeks of gestation with isoimmune AV block presented at 34 weeks with a precipitous fall in ventricular rate and periods of tachycardia. Magnetocardiography revealed the tachycardia to be ventricular. After delivery, nonsustained ventricular tachycardia continued. The baby then successfully paced, and at higher ventricular rates the tachycardia resolved. Five yea...
متن کاملAtrioventricular Conduction Delay in Fetuses Exposed to Anti-SSA/Ro and Anti-SSB/La Antibodies: A Magnetocardiography Study
BACKGROUND The presence of anti-SSA/Ro and anti-SSB/La antibodies during pregnancy is associated with fetal congenital heart block (CHB), which is primarily diagnosed through fetal echocardiography. Conclusive information about the complete electrophysiology of the fetal cardiac conducting system is still lacking. In addition to echocardiography, fetal magnetocardiography (fMCG) can be used. fM...
متن کاملGiant fetal magnetocardiogram P waves in congenital atrioventricular block: a marker of cardiovascular compensation?
BACKGROUND Cardiogram signal amplitude is a key index of hypertrophy but has not been investigated extensively in utero. In this study, magnetocardiography was used to assess P and QRS amplitude in normal subjects and subjects with fetal arrhythmia. METHODS AND RESULTS The study cohort consisted of 68 normal fetuses and 25 with various arrhythmias: 9 reentrant supraventricular tachycardia (SV...
متن کاملAtrioventricular Conduction Delay in the Second Trimester Measured by Fetal Magnetocardiography
INTRODUCTION Fetal AV block in SSA/Ro pregnancies is generally not seen before 18-week gestation and onset is rare after 28-week gestation. If complete AV block appears, it is believed to be irreversible. The purpose of the study was to evaluate precise electrophysiological AV conduction from 18-week gestation onwards. PATIENTS AND METHODS 21 fetuses of pregnant women with collagen vascular d...
